Community Navigator / Case Manager – New Castle County, DE

 

The Columbus Organization has immediate openings for Community Navigators (Case Managers) for New Castle County in Delaware who will provide services and support to intellectually and developmentally disabled individuals and their families in a community-based setting.

 

Job Responsibilities:

  • Evaluating and addressing individual risks and unmet needs in order to maximize the health, well-being and safety of intellectually and developmentally disabled participants
  • Assisting participants in coordinating all service needs whether Medicaid reimbursed, services provided through other funding sources, or those performed by natural supporters in the context of family or community life
  • Assisting participants in coordinating all service needs and have the responsibility to advocate, educate, train and guide individuals and families through the various avenues of information and communication around State/Federal policies, procedures and protocol
  • Participating in assessment of individuals through assembling both professionals and non-professionals who provide individualized supports and whose combined expertise and involvement ensures that person-centered plans are developed to address social, educational, transportation, housing, nutritional, healthcare and other needs using a holistic approach
  • Advocating for, encouraging and facilitating the use of various community resources through referral and follow up activities

Requirements:

  • Minimum of an Associate’s Degree or higher in behavioral, social sciences or a related field OR
  • Minimum of 1 year of experience in health or human services support, which includes interviewing individuals and assessing personal, health, employment, social, or financial needs in accordance with program requirements;
  • Have demonstrated ability to establish and maintain supportive relationships;
  • Have demonstrated experience and competency in supporting individuals and their families;
  • Have demonstrated experience in conducting person-centered assessments;
  • Have demonstrated skills in person-centered approaches to service plan development
  • Be skilled in conflict resolution strategies
  • Be skilled in leading team meetings;
  • Have knowledge of community resources and services
  • Previous Delaware case management experience a plus.
