Case Management in South Carolina
In collaboration with the State of South Carolina Department of Disabilities and Special Needs (SC DDSN), The Columbus Organization provides Case Management (Support Coordination) Services to individuals with developmental disabilities enrolled in the Intellectual Disabilities/Related Disabilities (ID/RD) waiver.
The Columbus Organization is also a provider of Case Management Services to individuals receiving the Head and Spinal Cord Injury (HASCI) waiver and Pervasive Developmental Disorder waiver which includes Autism or Asperger’s Syndrome.
Services
The Columbus Organization’s system of treatment monitoring and service utilization is a collaborative, cohesive process based on the needs of the individual and family, ensuring efficient and effective supports with positive outcomes. Our model of service delivery includes comprehensive, multi-systemic treatment planning and service coordination for each client. You decide how involved you would like your Care Coordinator to be in your life. Care Coordinators provide support through a variety of activities and work closely with you to help you:
- Fulfill your hopes, dreams, wants and needs
- Make decisions about the types of services you need and how you will receive them
- Decide what and how much information you want and how to get it
- Access Medicaid and non-Medicaid funded services
- Plan for the future – next month, next year, even ten years from now
- Use your creativity to achieve your goals, even when specialized services are not available
- Plan timelines for getting things done and deciding who should do them such as sharing information, calling resources for help, delivering services and getting needed equipment
- Make and coordinate appointments for services and supports
- Locate, enroll in and check on services for you or your family member
- Plan for changes in services or programs by discussing options, visiting new programs and preparing you, your family and the new service providers for the change
If your needs change we make sure the services and supports are changed as well, with input from the team working together. We pay close attention to outcomes and achievement of concrete, measurable goals to maintain successes over the long term.
The outcome of this treatment monitoring process is a person-centered, integrated individualized plan based on the person’s prioritized life domain needs.
