The Columbus Organization provides person-centered and choice-based support coordination services modeled on the principles of self-determination. Our focus is on providing individuals with intellectual and developmental disabilities the opportunity to lead a self-directed life with improved employability, relationships, and ability to make positive life decisions. Waivers:
Case Management Service is provided to individuals receiving Supports Waiver Program (SWP).
The Columbus Organization also provides Case Management services to individuals enrolled Developmental Disabilities Waiver Program (DD).
